Issues related to Kala Azar have been reviewed and it suggests that current treatment strategies are poorly selective, highly toxic and evidently have poor patient compliance and efficacy. Currently, AmBisome ® (Astellas Pharma US, Inc.) is the only Drug Delivery Systems (DDS) used against kala azar, and most experimental development only relates to parenteral administration. Amphotericin-B loaded Niosomes and Nanodisks, Cochleates containing AmB, mixed micellar system using Miltefosine are still under clinical trials. However, most of the DDSs in the pipeline remain as injectable formulations that require the use of discarded or sterilized material, trained personnel for administration and to deal with complications such as infections and potential allergic reactions to high doses of lipids. To employ friendly routes of adminsteration and at the same time allowing the modification of biodistribution and intracellular transit, new materials bringing new properties will be required since modification of a drug’s chemical structure will be insufficient. Hopefully, nano medicine can contribute to the development of new and more efficient strategies. Nanotechnology can provide the means and materials to build nano-DDSs in the size range (2 – 100 nm), prepared from small biodegradable natural or synthetic blocks, preferably by cheap bottom-up (self-assembly) techniques. Novel nano drug delivery systems for potential use against Kala Azar involve Dendrimers, Solid and Nanostructured Lipid Carriers, Nanoemulsions, Ultradeformable liposomes. Hence, nanotechnology presents a new hope for kala azar treatment in the era where the friendly routes of administration are difficult to be employed and the where the successful therapy is limited by a numerous side effects.
