EVALUATION OF SAFETY AND EFFICACY OF TOPIRAMATE IN THE PROPHYLACTIC MANAGEMENT OF CHRONIC MIGRAINE IN NORTH-EASTERN NIGERIA.

Abstract

Topiramate is an anticonvulsant agent with proven efficacy in migraine prophylaxis in many parts of the world especially the developed countries. This study aimed to evaluate the efficacy and safety of treatment with topiramate in patients with episodic and chronic migraine in north-eastern, Nigeria. Two hundred consecutive adult (18 years and above) patients that met the International Headache Society diagnostic criteria for migraine and has attended the Neurology Clinic of the Department of Medicine, University of Maiduguri Teaching Hospital, Maiduguri from March, 2009 to April, 2011 and from whom informed consent was obtained were evaluated for this disorder. The success of prophylactic therapy using topiramate tablet was prospectively studied at which their efficacy and safety was evaluated. Of the 158 patients that successfully completed the study, 6 (3.8%) and 12 (7.5%) experienced a dose dependent reduction in attack frequency of less than or equal to 24% and 25-49% respectively. On the other hand, 95 (60.1%) had a reduction in migraine attack frequency of 50-74%, whereas the remaining 45 (28.5%) experienced a reduction in attack frequency of greater than or equals to 75%. The most common dose dependent but self limiting adverse effects observed among the patients were paraesthesia, weight reduction, dizziness, insomnia, somnolence, amnesia and anorexia. Topiramate has significantly reduced the frequency, duration and severity of migraine attack with limited dose dependent adverse effects. Therefore, it is appropriate to consider topiramate as a first-line drug in migraine prophylaxis.
